Band 6: £37,898 to £45,637 a year per annum  PERMANENT  GOOD SALARY 

The position involves directly supporting and advising patients with palliative care needs in a hospital setting predominately as part of a MDT team.

An exciting opportunity has arisen to join our specialist palliative care team. The 37.5hour is a Band 6 clinical nurse role based in Y Bwthyn Royal Glamorgan Hospital Specialist Palliative Care unit. The position provides an outreach service to patients with palliative care needs across the hospital setting. The service provides support for the population of Rhondda and Taf Ely and patients who attend the hospital.

The position involves directly supporting and advising patients with palliative care needs in a hospital setting predominately as part of a MDT team. The core hours are Monday to Friday 9-5pm but depending on existing experience or knowledge gained during the undertaking of the role participation in a 7 day roster with the same working hours which will require giving advice to other health care professionals, patients and families, potentially visiting patients in the hospital and community setting at weekends is part of the role.

The successful applicant would be responsible for managing a caseload of patients with specialist palliative care needs in collaboration with the wider MDT, participating in an education, audit and providing leadership regarding the specialty.

You will need to be enthusiastic about the specialty, demonstrate experience of caring for patients with palliative care needs and be able to meet the travel requirements.

The Specialist Palliative Care Team in CTMUHB is a multi-professional team providing a clinical, advisory and educational service across CTMUHB. The team are situated in locality bases which each have a SPC inpatient unit and house services that provide outreach services to both community and hospital patients. An appreciation that on occasions, dependent on patient need there may be a need for post holder flexibility in providing support to cover the CNS role across other sites.

The candidate will be able to support the CNS to assess and treat palliative patients in their own homes and offer advice and support . Liaising with District Nursing the candidate will be able to offer support and guidance to patients, families and carers.
