An exciting full-time opportunity has arisen to develop the Band 6 clinical Pharmacist role in our patient focused pharmacy service, based at Moseley Hall Hospital and West Heath Hospital.
As a qualified and registered clinical Pharmacist you will specialise in Elderly Care, joining our team of Pharmacists and Technicians providing medicines optimisation support and advice to our adult in-patient wards.
You will have the opportunity to experience a variety of areas and continue to challenge yourself to grow as a Pharmacist.
Our team endeavours to provide a comprehensive, efficient and safe Pharmacy service to all clinical areas. This is to ensure we are consistently contributing to quality improvements in both prescribing and overall patient care.
We are looking for a friendly, dedicated and motivated Pharmacist to join our expanding team.
Working alongside clinical colleagues you will be integrated in to the ward teams, attending multidisciplinary meetings and consultant ward rounds to support the pharmaceutical needs of patients under your care.
You will have excellent communication and interpersonal skills, in addition to being able to organise your workload to prioritise your time. You will also have the ability to work under pressure, whilst ensuring that all patients experience out Trust values whilst in our care.
The ideal candidate will be willing to learn and would be looking to start a post-graduate clinical diploma.
An on-call commitment will be part of the role.
We are a small, friendly forward thinking team that is currently expanding, providing a ward based Pharmacy service. Patient experience and safety is at the centre of everything we do.
About us
Support, mentoring and further training will be provided to the successful candidate by the Head Pharmacist and team members, to ensure that you fulfil your potential in this new challenging role. You will be actively encouraged to contribute to service development and quality improvement. This will provide the opportunity to develop leadership and managerial skills.
- Dispensing and checking of prescriptions & issuing medication from the Pharmacy and aiding patient discharges, including competent in the use of the computer system. Act as the responsible Pharmacist for the dispensary as allocated.
- To clarify poorly written prescription charts, discharge letters and other forms for prescribing. Challenging inappropriate prescribing.
- To query irrational or inappropriate prescribing, including unlicensed, off-licence and non-formulary prescribing. Record clinical interventions and activities in line with departmental policy.
- Document patient care contributions in the patients history notes and use the trusts incident reporting system to report any medication errors.
- To support the Head Pharmacist in maintaining an efficient and safely operating Pharmacy service and drug supply service to the wards.
- To help maintain and develop a clinically orientated Pharmacy service to ward and outside units. Formulate individual patient pharmaceutical care plans. Participation in Consultant wards rounds.
- To ensure the efficient supply of pharmaceuticals is maintained to the wards.
- To supervise Pharmacy Technicians as appropriate.
- Provision of drug information as necessary utilising the Drug Information Departments at local NHS Trusts, as appropriate.
- Interpretation of therapeutic drug level results and provision of professional advice where needed.
- To help support the self-administration of medication scheme in operation at both hospitals under the supervision of the Head Pharmacist. Education of patients about their drug therapy.
- To help support discharge counselling at both hospitals under the supervision of the Head Pharmacist.
- To co-operate with objective setting, individual personal assessment and continuing education and training programmes, in order to ensure continuing professional development and competence to practice as required by clinical governance.
- To attend and participate in Pharmacy clinical meetings.
- To promote the formulary for antimicrobial stewardship, educational and drug economy purposes.
- To help support a pharmaceutical educational programme for Nurses on each site under the supervision of the Head Pharmacist.
- To help support the medicine reconciliation service on the wards. Review patients medication to ensure safe and effective use
- To advise on the development of guidelines and Standard Operating Procedures for Pharmacy and wards.
- To cover leave of the Pharmacists at each hospital.
- To promptly act on drug alerts and drug recalls.
- To support mandatory medicines management training of clinical staff. Provide information to medical staff, other healthcare professionals and members of the public regarding medicines.
- Any other duties as required to maintain and develop the Pharmacy service.
- To comply with the requirements of National Statue and local policy, including the Medicines Act, Misuse of Drugs Act, Health and Safety at Work Act etc, so that work done in the department is within recommended and legal parameters.
- To attend meetings in the Pharmacy as necessary in order to fulfil job functions.
- May need to provide on-call Pharmacist duty within the on-call rota.
- Undertake any other duties commensurate with this grade of post in agreement with the relevant line manager
