The Transition Case Manager role at Aneurin Bevan University Health Board in Newport involves managing the transition of individuals with complex needs into adulthood, specifically within the Mental Health and Learning Disabilities Commissioning Team. The position focuses on ensuring a seamless provision of integrated services for those eligible for continuing healthcare (CHC) and who may require additional commissioned services. The Transition Case Manager will gather relevant information to understand the needs of individuals, work collaboratively with other professionals, and maintain strong links with Community Mental Health Teams (CMHTs) and Child and Adolescent Mental Health Services (CAMHS).

Key responsibilities include case management for a caseload of patients, implementing the National Framework for Continuing Healthcare, and adhering to The Transition and Handover Guidance.
