With progression from band 5 to band 6 expected within 6 to 12 months dependent on achieving competencies. You would be joining a supportive and expert multidisciplinary community team, following a structured competency programme under the guidance of clinical nurse specialists.
In this role you will bring your enthusiasm to learn and develop within palliative and community care. Delivering high standards of care, working as part of a multidisciplinary team providing advice, psychological support and symptom control for patients with an associated life limiting condition and their families, within both home and out patient settings.
The successful candidate will be part of the Integrated Community Team and will work closely with the multi-disciplinary team and other care providers, working as a community nurse under the direction of the specialist nurses.
You will provide advice and support to patients affected by life-limiting disease and support their families. At the patients home or other care settings and in our out patient setting, as required. Under the guidance of clinical nurse specialists you will assess patients physical, psychological, spiritual and information needs, plan, implement and evaluate care, provide specialist symptom control advice and emotional support.
