Whittle Surgery is seeking an Advanced Care Practitioner to join their multi-disciplinary team for 24 hours a week, from Wednesday to Friday. The role encompasses patient care through consultations both in the practice and in home settings, as well as administrative duties such as reviewing hospital correspondence and lab results. The practitioner will operate independently, using advanced clinical skills to assess, diagnose, and treat a variety of medical conditions, ensuring proper referrals to specialists and coordinating care within the team.

The position also involves program management to develop specialized care plans, conduct health screenings, and contribute to service improvement initiatives. Strong communication skills are essential for interacting with patients, families, and colleagues, often in sensitive situations. Additionally, the practitioner will play a leadership role, mentoring new staff, participating in clinical meetings, and engaging in research and audit activities to enhance clinical practice within the surgery.
