St Luke's Hospice in Basildon is seeking a part-time Community Nurse Team Leader to oversee a team of community nurses providing high-quality palliative care to patients and their families. This role involves leading and guiding the team to ensure timely and appropriate care delivery, collaborating with other health professionals, and participating in audit and research projects relevant to the service. Additionally, the team leader will monitor and evaluate team performance, fostering a culture of continuous improvement and professional development within the team.
St Luke's Hospice in Basildon has a fantastic opportunity for a community nurse team leader, on a part-time basis for 22.5 hours per week.
You will manage a team of community nurses ensuring the provision of high standards of care to patients and their families or carers who have a palliative care need.
- Providing leadership and guidance to the team in Hospice Community Services.
- Supporting the teams to ensure the delivery of the right care at the right time to individuals with a palliative care need in the community.
- Working, actively supporting and liaising with other health care professionals and agencies.
- Assisting in audit and research projects, particularly within the service, taking a lead on specified projects as appropriate.
- Monitoring and evaluating team performance, providing guidance and support where necessary.
- Fostering a culture of continuous improvement and professional development within the team.
