Job Summary: Community Nurse - Band 5 at NELFT in Basildon
NELFT is looking for a motivated and dynamic Band 5 Community Nurse to join the Integrated Care Team in Basildon & Wickford. The role involves working collaboratively within a community-based team that includes physiotherapists, occupational therapists, nurses, and care coordinators, providing holistic care to patients.
Key Responsibilities:
  • Manage and deliver care for assigned patients.
  • Assess, plan, and implement care strategies.
  • Support peers and supervise nursing students and assistants.
  • Collaborate effectively with other health and social professionals.
  • Maintain electronic patient records.
Requirements:
  • Registered Nurse with NHS experience; community experience is desirable but not essential.
  • Ability to work independently and as part of a team.
  • Computer literacy is necessary.
