The West Herts Community Heart Failure Service is seeking a Band 7 Specialist Nurse, Clinical Pharmacist, or Paramedic for a 6-month fixed-term position in Brickett Wood, Watford. The successful candidate will have advanced clinical skills and experience in managing cardiac conditions, working within a well-established Heart Failure Team that collaborates closely with acute partners and other community services. The role includes managing a caseload of heart failure patients in various settings, supporting the Heart Failure Virtual Hospital, and implementing innovative care pathways to enhance patient outcomes.

Candidates must possess excellent communication skills, be self-motivated, and an effective team player while working with GPs and integrated community teams. A valid UK driving license and access to a car for business purposes are required.
