The role of "Vulnerable Children and Young People’s Wellbeing Support Practitioner" at Mid Cheshire Hospitals NHS Foundation Trust involves a blend of clinical and administrative responsibilities aimed at supporting the wellbeing of children and young people (CYP) facing mental health and behavioral challenges. The practitioner will collaborate closely with the Vulnerable CYP Specialist Nurse and the multidisciplinary team (MDT) to enhance care pathways, conduct risk assessments, and coordinate services during the absence of the specialist nurse. Key tasks include improving communication with patients and families, contributing to audits, and maintaining awareness of safeguarding protocols, with the goal of minimizing health inequalities for vulnerable CYP.

Candidates should possess strong communication, organisational, and interpersonal skills, alongside an understanding of NHS policies and a commitment to quality care. Responsibilities also include fostering links with support teams, managing administrative tasks, and ensuring compliance with Trust guidelines while being adaptable to prioritize workload demands. Ultimately, the role offers an opportunity to make a meaningful impact on the lives of vulnerable children and young people within a healthcare setting.
