Earnswood Medical Centre in Crewe is seeking an Advanced Nurse Practitioner to join its busy and committed team. The ideal candidate should possess a flexible, can-do attitude and a strong dedication to primary care values. The role involves working within a total triage system, triaging and treating patients through face-to-face consultations and telephone assessments. Responsibilities include diagnosing and managing both acute and chronic conditions, prescribing and reviewing medications based on evidence-based practice, and coordinating care plans while ensuring patient involvement.

The position requires the ability to refer patients to both primary and secondary care professionals, manage diagnostic investigations, and engage with a multi-disciplinary team to ensure comprehensive patient support and care delivery.

We operate a total triage system and the successful candidate would be expected to work in the triage team through a rota system with other members of the ACP team.
Main duties of the job
The candidate should be able to demonstrate that they can
- Triage and treat patients (both face to face and via telephne), performing specialist assessment of their needs and make referrals as necessary to other members of the primary and secondary health care teams as appropriate.
- Prescribing and reviewing medication that is appropriate to patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.
- Diagnose and manage acute and chronic conditions, including drug and non-drug based treatment methods into a management plan.
- Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc.
- Assess, diagnose and treat patients in practice premises or their own home who require medical attention and/or refer to appropriate agency.
- Refer patients to other members of the multi-disciplinary practice teams, secondary care organisations and other organisations as necessary.
