MENTAL HEALTH SOCIAL WORK
The Crisis Resolution Home Treatment Team Practitioner role with Powys Teaching Health Board involves working within a team to deliver a Crisis Resolution Home Treatment (CR/HT) service for adults facing mental health crises and at risk of hospitalisation. The position requires providing intensive home treatment in a collaborative environment, available 7 days a week year-round, including shift work comprising 12-hour and 8-hour shifts. The role is aimed at supporting individuals in acute mental health distress, facilitating early discharge from inpatient units, and ensuring care is provided in the least restrictive setting.

Candidates should be qualified practitioners—either a Nurse, Social Worker, or Occupational Therapist—with a background in community mental health work and a strong passion for this field. The position is based in a supportive team with low staff turnover in a picturesque rural setting, and while the ability to speak Welsh is desirable, English speakers are also encouraged to apply. The job offers opportunities for career progression from band 5 to band 6, with flexibility for job share or part-time hours considered by the team manager.
