The Liaison and Diversion Senior Practitioner (Bank) Band 6 role at South West Yorkshire Partnership NHS Trust involves providing specialized assessments and support to individuals with vulnerabilities in police custody and court settings. The service is designed to address mental health, substance use, homelessness, learning disabilities, and other challenges faced by those caught in the youth or criminal justice systems. The practitioner will work autonomously to assess individuals, facilitate their diversion to appropriate services, and collaborate closely with multi-disciplinary teams and partner agencies to ensure tailored care pathways. Key responsibilities include conducting mental health assessments, providing timely information to support case disposal, and actively engaging with various stakeholders to optimize the wellbeing of clients.

Additionally, the practitioner will supervise junior staff, maintain client records, and contribute to policy development and clinical audits within the Trust, all while promoting evidence-based practices and participating in ongoing professional development opportunities.
