The Confederation, Hillingdon CIC, is seeking a motivated and innovative Clinical Pharmacist to join the Celandine & Metrocare Primary Care Network (PCN) in Uxbridge on a permanent basis. The successful candidate will collaborate with GPs across multiple practices to enhance medication management and deliver patient-facing clinical services. Responsibilities include conducting clinical medication reviews, managing long-term conditions, developing safer prescribing systems, and providing medication support to patients. The role requires a strong understanding of pharmacy practices, and the ideal candidate should possess an undergraduate pharmacy degree, a minimum of two years of experience, and be committed to ongoing professional development.

Key functions of the position involve engaging with patients to optimize their medication use, reducing the risk of hospital admissions related to medications, and ensuring continuity of care post-discharge from hospitals.
