The Community Mental Health Team is looking for a motivated and organized Community Mental Health Nurse to join their multi-disciplinary team in Denbigh, focusing on individuals with severe and enduring mental health issues. The role involves care coordination and case management for a designated caseload, adhering to the standards set by the Mental Health (Wales) Measure 2010. The nurse will be responsible for developing comprehensive care and treatment plans, conducting risk assessments, and providing support in clinical duties such as administering long-acting injections and running clinics for specific medications.

Additionally, the successful candidate will engage with service users, families, and partner agencies to deliver high-quality care, particularly in crisis situations. The ability to communicate in Welsh is desirable, although applications from English speakers are also welcome. This position is crucial in ensuring that patients receive appropriate care tailored to their complex mental health needs.
