Band 7: £46,148 to £52,809 a year  PERMANENT 
The post holder will support the virtual ward registered staff and clinical team ensuring the delivery of high-quality patient care upholding patient dignity delivered with kindness and compassion.

Nottingham City Care is looking for an innovative, enthusiastic, and motivated individual to join our existing VW Matron within the virtual ward team within Nottingham City.

Our Virtual Ward Matrons are an integral part of our virtual ward providing Leadership, clinical expertise and knowledge to our patients and fellow clinicians. The aim is to reduce hospital admissions and facilitate discharge.

The Virtual ward provides clinical care and support for to patients who have an acute illness, but can be offered additional support, under the care of a medical consultant/GP, to enable them to be treated in their own homes, thereby preventing an unnecessary hospital admission. The Virtual Ward team are also able to provide support to patients who may be medically unwell but stable allowing them to be discharged back to their own homes earlier reducing the number of days required in hospital. The patients will remain on the virtual ward, under the care of a medical consultant/GP for up to 14 days.

The core hours of the virtual ward are 8am to 8pm, 7 days a week.

You will be part of the wider multi-disciplinary team, working in collaboration with consultants, senior nurses, assistant practitioners, GPs, Physiotherapists, OTs, Pharmacists, secondary care, and social services.

The Community Matron will be an Independent prescriber and the successful candidate will either hold the V300 qualification or be ready to undertake it within 12 months in role.

You will:

  • Provide clinical leadership to the VW team.
  • Be an Independent prescriber (V300)
  • Carry out physical examinations.
  • Provide intense clinical input and monitoring to acutely unwell patients.
  • Use your expert knowledge and clinical judgment to identify the potential diagnosis and where appropriate make a final diagnosis.
  • Use your extensive practice experience to plan and provide skilled and competent care that meets patients' health and social care needs, involving other members of the healthcare team as appropriate.
  • Ensure the provision of continuity of care, including follow-up visits.
  • Assess and evaluate with patients, the effectiveness of the treatment and care provided and make changes as needed.
  • Work independently, although often as part of a healthcare team.
  • See patients in their own home and ensure that each patient's treatment and care is based on best practice.
  • Work as part of multi-professional and multi-agency teams.
  • Provide supervision, education and support to patients and colleagues as appropriate.
  • Support the Clinical Service Manager in operational management of the team.

Virtual wards support patients, who would otherwise be in hospital, to receive the acute care, remote monitoring, and treatment they need in their own home or usual place of residence. Virtual wards provide acute clinical care at home for a short duration (up to 14 days) as an alternative to care in hospital. The overriding purpose of the community matron is to support the provision of the highest quality patient care to individuals whilst on the virtual ward pathway, supporting the service manager.

Have a clinical leadership role, with responsibility for planning, managing and co-ordinating the care of people with highly complex needs and long-term conditions within a defined caseload.

The Virtual Ward core hours are 08.00 - 20.00 over 7 days.


Role Context:

  • To provide appropriate and effective care management through the virtual ward pathway unnecessary hospital admission from the Community managing the active caseload and providing necessary oversight and support
  • To holistically assess the needs of patients and carers, plan, implement and evaluate nursing care interventions.
  • Embrace multi professional and partnership working to facilitate co-ordination, promotion of optimum health and social outcomes independence and self-care.
  • Undertake virtual monitoring and escalate concerns to the daily MDT.
  • Will be responsible for holistic assessments, implementation of care, overall co-ordination of self-care management or care management.
