The Dudley Group NHS Foundation Trust is seeking an experienced and motivated Clinical Triage Nurse for its Single Point of Access team at the Brierley Hill Health and Social Care Centre. This role involves providing telephone triage, clinical assessments, and signposting for patients registered with Dudley GPs to support urgent care and avoid unnecessary hospital admissions. The service operates daily from 8 am to 9 pm, and the nurse will be expected to work flexibly during these hours. Key responsibilities include taking referrals from various health professionals, collaborating with the Urgent Community Response Team, and visiting patients in their homes when necessary.

Candidates should possess a strong clinical background, excellent communication and IT skills, and the ability to prioritize workloads effectively. The successful applicant will provide support to healthcare professionals, challenge decisions when necessary, and play a crucial role in facilitating multi-disciplinary team management to deliver appropriate care closer to home. The position requires individuals who are friendly yet assertive, capable of making confident decisions, and eager to contribute to a proactive and collaborative healthcare environment.
