The Access to Community Services Team (ACST) at Wrightington Wigan & Leigh Teaching Hospitals NHS Foundation Trust is seeking a full-time practitioner—either a nurse, occupational therapist, or physiotherapist—to facilitate safe and early discharges from the Emergency Care Centre. The role requires substantial NHS Band 5 experience and an understanding of handling patients with complex medical, functional, and social issues. The successful candidate will conduct comprehensive assessments, provide advice on community services, and contribute to a multidisciplinary approach in patient care, while also engaging in service improvement initiatives.

Additional responsibilities include planning and organizing patient management, participating in training and education for healthcare professionals, maintaining accurate patient documentation, and supporting the management of patient care pathways to prevent unnecessary hospital admissions. The role emphasises effective communication and collaboration with patients, family members, and other healthcare providers, while also necessitating ongoing professional development and adherence to health and safety guidelines.
