The Hampshire and Isle of Wight Healthcare NHS Foundation Trust is looking for an Extended Clinical Practitioner (ECP), either a Nurse or Allied Health Professional (AHP), to join their Urgent Community Response (UCR) and Frailty Virtual Ward (VW) team in Basingstoke. The ECP will provide high-quality, complex care to patients in their homes, aiming to prevent avoidable hospital admissions by delivering advanced clinical assessments and treatments within two hours of referral. Collaboration with a multi-disciplinary team, including senior clinical practitioners and community healthcare professionals, is essential for developing and evaluating patient care plans.

The position requires strong clinical judgment, analytical skills, and the ability to adapt to dynamic healthcare environments. The ECP will engage with patients and their families, undertake comprehensive assessments, and implement complex interventions while maintaining accountability for care delivered by the team. The role involves flexible working hours, including at least one weekend shift per month, as the service operates 365 days a year from 0800 to 2000.
