To offer an extended service to ensure an appropriate assessment of mental health need for CYP in mental health crisis and in need of appropriate intensive support ensuring the delivery of safe and effective practice as part of the CYP wellbeing and recovery action plan. The post holder will work directly with children and young people with mental health needs and their families, carrying out assessments (including risk management) and effective treatment and/or interventions, with the aim of helping children and young people participate in normal family and community activities and to facilitate positive outcomes of care in relation to young people's mental health needs. The role will also facilitate hospital admission avoidance and seamless transitions between services where appropriate.
They will also be expected to work flexibly within the Team to support a crisis, brief intervention and intensive support function. Dependent on skill and service need they will need to be available to external professionals and carers to offer advice, signposting and guidance (especially in relation to urgency and risk) associated with children and young people's mental health and emotional wellbeing.
Potholder will provide assessments and interventions for CYP as part of the MDT for those experiencing severe to moderate mental health conditions.
Mental Health, Children's and LD Nurses and those with a social work background are encouraged to apply.

As part of this role, you will be required to travel across wide geographical area and have access to a car, with valid driving licence and business insurance.
