The Community Mental Health Practitioner role at Midlands Partnership NHS Foundation Trust in Lichfield involves providing comprehensive assessment and support for children and young people (CYP) experiencing mental health crises. The practitioner will work directly with CYP and their families to conduct assessments, manage risks, and deliver effective treatment and interventions. The primary goal is to enable CYP to engage in family and community activities while facilitating positive mental health outcomes, including avoiding hospital admissions and ensuring smooth transitions between services.

The position requires collaboration with a multidisciplinary team (MDT) and demands flexibility to support crisis situations, brief interventions, and intensive support functions. Additionally, the practitioner will provide guidance and advice to external professionals and caregivers regarding mental health concerns. Candidates must possess valid NMC, Social Work England, or HCPC registration and be prepared for extensive travel within the service area.
