- Administrator for setting up patients and clinicians on to the COPD Virtual Ward.
- Provide digital training to patients at home.
- Support patients with technical queries and issues that the patient or clinician has identified using the equipment.
- Support monitoring compliance of the digital platform as part of the COPD Virtual Ward and liaise with patients and clinical staff.

- Recognising issues relating to safeguarding vulnerable adults and report any problems or raise concerns to the Case Manager or Team Lead.
- Assisting in the assessment of and contribute to the management of risk across several areas within the patients home environment.
- Supporting the Programme Lead, with the planning, monitoring, and delivery of key milestones.

As part of the NHSE Virtual Ward agenda, this post will be the main Co-ordinator for the co-ordination, telecare monitoring and facilitator for the Virtual Ward Chronic Obstructive Pulmonary Disease (COPD) service. The role will be the main co-ordinator of COPD patients, identified to use digital tools to identify worsening symptoms of COPD so that early support and interventions can be made and avoid escalated deteriorating health which can often result in unnecessary hospital attendances and/or hospitalisation. The post holder will work alongside the CNS team to support patients identified for surgical intervention. The role requires travel across Stoke-on-Trent, North Staffordshire and Stafford.
- Clinical Care: Providing Home Visiting and Clinic support to the Specialist Nursing Team either collaboratively or independently. You will be able to utilise your existing clinical skills and have the opportunity to gain an accredited qualification in Spirometry.
- Utilising Digital Remote Monitoring (DRM) to manage a changing cohort of COPD patients in accordance with a structured triage process. The postholder will support COPD patients using digital tools to identify worsening symptoms of COPD ensuring early support and interventions can be implemented to avoid deteriorating health and unnecessary hospitalisation.
