Join Our Team as a Social Prescribing Link Worker in Ely!
Are you passionate about making a difference in people's lives? We are looking for a dedicated Social Prescribing Link Worker to join our supportive team at Priors Field Surgery in Sutton and St Mary's Surgery in Ely for 22.5 hours a week. In this rewarding role, you’ll help individuals navigate their health and wellbeing needs, connecting them to local community resources and providing practical support to enhance their quality of life. Whether it’s assisting with housing, social connections, or managing everyday challenges, your guiding hand will empower our clients to take charge of their health journey.
As a vital part of our GP Practice teams, you will work closely with colleagues and the local community to identify what support is needed, making a real impact in the lives of those around you. With much of your work done online and through phone calls, you’ll also have the opportunity to meet clients in person, forging meaningful connections and helping them find solutions tailored to their unique needs. If you’re a skilled communicator who understands the importance of community, we want to hear from you!
