A qualified nurse who will support the team lead in ensuring that palliative patients receive the highest standard of care. To act as a role model and support the team manager to operationally manage and lead a team of carers who provide care to patients in their own homes. To be proficient in IT skills and to ensure the management of rotas and support the RPCS team manager with monthly reporting requirements with accurate recording of data.
To support the team lead in clinical assessment of patients and the management of the caseload.
To support and act as a mentor for carers and liaise with members of the health and social care professional, patients, families and carers.
To ensure accurate recording of data, management of daily rotas and to be part of the out of hours call rota.
To work flexibly across the organisation Acting as a Rennie Grove Peace ambassador within the community
To be responsible for the line management of an agreed number of carers, allocation of shifts and rota management, 1:1 meetings, Individual Progress Review (IPR), assessment of competencies.
Support the manager with regular staff meetings To monitor and report incidents.
To work closely with the Head of Community Services and the Team manager in delivering the service provision and undertaking any operational projects to support the RGP strategy and relevant contracts.
Attend relevant meetings as appropriate.
