The Sussex Community NHS Foundation Trust is seeking an enthusiastic and motivated Community Staff Nurse to join their established team serving Heathfield, Uckfield, and surrounding areas. The role involves providing high-quality, holistic nursing care to patients in their homes while collaborating with a multidisciplinary team to enhance integrated health and social care. Key responsibilities include conducting holistic assessments, creating individualized care plans, promoting self-care, and supporting patients to remain at home. The position offers ongoing training and professional development opportunities and requires a recognized qualification in Adult Nursing, a UK driving license, and access to a vehicle.

While previous experience in a community setting is desirable, new graduates are welcome, as comprehensive induction and training will be provided.

We are looking for a dynamic, enthusiastic and motivated individual to join our workforce in a busy and well established community team. Greater Wealden PCN provides nursing services to Heathfield, Uckfield and surrounding area. Our community nurses benefit from having continual opportunities to develop an excellent skillset and become autonomous practitioners whilst working as part of a team delivering high quality, holistic care to patients in their own homes.

Primary Care Networks (PCNs) are an innovative service development involving community nursing. The ambition is to integrate multidisciplinary professionals so health and social care can be delivered seamlessly to those who need it. Due to this, community nursing offers a dynamic nursing environment which enables nurses to become confident, skilled practitioners.

About us
Successful applicants will have a recognised qualification in Adult Nursing, a UK driving license and a car. Prior experience in a community setting is desirable, although a full induction and training will be provided. We are also able to support newly qualified nurses.
