The role of Community Mental Health Practitioner with Midlands Partnership NHS Foundation Trust in Lichfield involves providing intensive support to children and young people (CYP) experiencing mental health crises. The practitioner will conduct assessments, manage risks, and deliver effective treatment and interventions aimed at helping CYP engage in family and community activities while facilitating positive mental health outcomes. Additionally, the role emphasises hospital admission avoidance and smooth transitions between services, requiring flexibility to support crisis situations and collaborate within a multidisciplinary team (MDT).

Candidates with backgrounds in Mental Health Nursing, Children's Nursing, Learning Disabilities Nursing, or social work are encouraged to apply.
