The Senior Mental Health Practitioner role at Mersey Care NHS Foundation Trust in Huyton involves two positions within the Knowsley Community Mental Health Team (CMHT) for registered mental health nurses. The primary responsibilities include conducting comprehensive biopsychosocial assessments of patients with moderate to severe mental health conditions, providing specialist advice and support, and facilitating timely access to mental health services. The role emphasises a collaborative, multi-disciplinary approach, enabling the practitioner to effectively triage and direct patients to the appropriate teams or services.

Working primarily during weekday core hours, the practitioner will engage in clinical evaluations, monitor patient adherence to treatment, and manage sensitive information with families and caregivers. The position also involves community resource identification, short-term intervention for patients, and participation in quality improvement initiatives.
