The Community Assistant Practitioner - Therapy role at Norfolk Community Health and Care NHS Trust in Kings Lynn involves working within The Intermediate Care team to provide rehabilitative care to patients with various conditions. The practitioner will conduct patient assessments, set goals, and implement therapy treatment plans under the guidance of qualified Occupational Therapists and Physiotherapists. Home visits and active participation in discharge planning are integral aspects of this role.

Candidates for this position should possess a current UK driving license and have access to a vehicle, as they will be required to travel to various locations. The role requires independent working on specific tasks while also receiving regular clinical supervision. Additional duties include documenting patient visits, managing communication with patients and their families, and maintaining high standards of infection control.
