The Dene and Stour Valleys Primary Care Network is seeking a proactive Care Coordinator to join their Wellesbourne team in Leamington Spa. This role, part of an evolving position within NHS Primary Care Networks, requires a flexible individual with a good understanding of the community or primary care sector and strong IT skills. The Care Coordinator will facilitate patient support before and after clinical consultations, coordinate key activities to enhance service access, and collaborate with Social Prescribing Link Workers to address local health priorities and patient needs.

The successful candidate will manage patient navigation across health and care services, work with health and wellbeing coaches, and assist patients with mental health support. They will also support the organisation of multi-disciplinary team meetings, produce documentation, and ensure effective communication within the network. A clean UK driving license and vehicle are essential for this role due to travel requirements between Dene & Stour practices.
