We have a great opportunity for a secondary breast clinical nurse specialist role. You will provide specialist nursing advice and support to patients, diagnosed with a secondary breast cancer, and their relatives and carers.
The successful candidate will facilitate the development of the secondary breast cancer pathway and work collaboratively with the multidisciplinary teams within the breast unit and the oncology unit across the Trust.
The successful candidate will work closely with the ward staff and the wider multidisciplinary team members and liaise with primary and secondary team members to facilitate and co-ordinate patient centred care.
You will ensure patients receive all the relevant information to enable them to make an informed choice about their treatment. You will work with the patients, families and carers to assess patients needs (using the holistic needs assessment tool), developing individual care plans and to re-evaluate as the ongoing care needs change.
