The Hertfordshire Partnership University NHS Foundation Trust is seeking a qualified Community Mental Health Nurse for a Band 6 position in Watford, focusing on a new Low Intensity Psychosis Pathway within the Adult Mental Health Community Transformation Programme. The role involves leading a mini team responsible for providing comprehensive care to service users with low intensity psychosis, emphasizing personalized recovery and empathetic relationships. The successful candidate will conduct health needs and risk assessments, develop care plans, and collaborate with a multidisciplinary team to address the needs of service users, their families, and carers.
