We work in partnership with a number of hospitals across the country to collaboratively redesign outpatient services with more care being delivered in community settings.

We are looking for experienced community Healthcare Assistant who have the skills, knowledge and experience to assist the nursing team in delivering excellent patient care

This is a hands-on patient facing role; we are seeking individuals to join our team who are caring, compassionate and confident, who demonstrate effective communication, excellent customer service skills, good IT skills and the ability to support patients with interventions and/or queries. The postholder will be a key member of the practice team, responsible for undertaking patient health checks inc. BP / ECG monitoring / height and weight, taking blood samples, health and wellbeing advice and promotion.
