The Post holder will provide specialist School Nursing Services for children and young people with learning and physical disabilities within special schools, utilising a "Team around the Child" approach and demonstrating excellent evidence-based health care interventions.

Work with Special Schools, families and the multidisciplinary team to promote the health and wellbeing of the children and young people within the educational settings.

Acting as the key professional, the post holder will coordinate and facilitate the sharing of all health-related information.

Within the Children's Community Nursing and Special School Nursing Team, the post holder will have direct responsibility for the management of the designated special needs school and tasks as required.

Ensure delivery of universal, enhanced and intensive levels of care while being aware of the need for using "reasonable adjustment" to achieve positive outcomes and help the children reach their full potential.

Provide advice regarding medication management, record keeping, and quality assure relevant policies and procedures which relate to the child's health and wellbeing needs.

Work in partnership with children, parents and carers, together with other professionals and agencies to meet health, development and safeguarding needs.

Alongside School, the post holder will develop specialist health care plans to ensure healthcare is delivered in a timely manner.

Facilitate the provision of effective, efficient and appropriate high-quality care to special needs school children and young people ensuring the highest standard of nursing care is provided, monitored and evaluated. Ensure colleagues, School staff and students feel valued and supported through the provision of training, supervision and assessment.

Be competent to manage any assistive technology, aids and equipment.Assess, plan, implement and evaluate the holistic nursing and health care needs of children and young people with a range of conditions from acute illness to complex, life limiting and end of life care.

Provide expert knowledge, strong clinical leadership, direction and development of staff in the designated special needs school setting, to improve the quality and shape services to meet Local and National Agendas and needs of individual communities in partnership with other agencies. Support the School to implement health care plans and minimize inappropriate hospital admissions.

Liaise with other services to co-ordinate and support young people's individual needs and choices within the special needs school environment. Function as an autonomous, competent practitioner in the assessment and delivery of high standards of care and treatment, acting as a principal nursing contact for familieswhose children attend the Special School.
