Individuals who have been diagnosed with Severe Mental Illness (SMI) are at much greater risk than those without SMI, of developing complex Physical Health conditions such as diabetes, respiratory problems, heart conditions and cancer. It is therefore essential that although we are a Mental Health trust, we also closely monitor the Physical Health of this vulnerable group of patients to ensure they receive appropriate care.
This role will involve supporting the SMI Physical Health Team for Norfolk & Waveney, in streamlining and restructuring appointment systems, alongside improving the referral pathways into our service and the follow up communication with our Primary Care colleagues. You will play a central part in helping to develop a well organised Team to achieve consistency of care across Norfolk and Waveney.
