The Leys GP Partnership in Oxford is seeking an Advanced Nurse Practitioner (ANP) with a focus on elderly patients, specifically to provide clinical services in six care homes within the South East Oxford Health Alliance. The role involves close collaboration with general practitioners, including a designated GP clinical supervisor, to manage patient care through chronic condition assessments, medication reviews, and addressing acute health issues.

Key responsibilities include conducting weekly ward rounds, developing and updating care plans, clerking new patients, and ensuring appropriate medication management. The ANP will also coordinate with various external agencies and support services to facilitate comprehensive care, particularly for end-of-life planning. Confidentiality, health and safety adherence, and support for equality and diversity are essential aspects of the role, with a commitment to maintaining accurate medical records and promoting best practices in healthcare delivery.
