Job Summary: Community Nurse – Hampshire and Isle of Wight Healthcare NHS Foundation Trust (Waterlooville)
Join the innovative Hampshire and Isle of Wight Healthcare Trust as a Community Nurse in the out-of-hours nursing service. The role involves working twilight (5 PM - 11:30 PM) and night response (10 PM - 4:30 AM) shifts, offering a part-time contract of 12 hours with unsocial hour enhancements, promoting an excellent work-life balance.
Ideal candidates will have community nursing experience, strong clinical and communication skills, IT proficiency (particularly with the Rio recording system), and the ability to work autonomously. The role requires managing a defined caseload, conducting holistic assessments, developing care plans, and ensuring high-quality patient care with a focus on preventing unnecessary hospital admissions and supporting community discharges.
