We are currently recruiting for an experienced Staff Nurse to join the out of hours nursing service. Working Twilight 17:00 until 23:30 and Night response 22:00 until 04:30 am. This position provides excellent work life balance, and We have a part time contract of twelve hours available which attract unsocial hours enhancements. We are looking for enthusiastic, caring and motivated professionals with excellent clinical and communication skills and the ability to work autonomously. IT skills are also essential as we use the Rio recording system. We are seeking an individual who has post registration experience of working in the community setting, due to the nature of this role as experience is often needed with the challenges of out of hours. The post holders must have relevant experience in health care and demonstrate a proactive and positive attitude to learning the skills required to fulfill the role. Open only to applicants from Hampshire & Isle of Wight NHS system, which is limited to: Hampshire and Isle of Wight ICB, Hampshire & Isle of Wight Healthcare FT, Portsmouth Hospitals University NHS Trust, University Hospital Southampton FT, South Central Ambulance Service or Hampshire Hospitals FT. Working within Professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload (via Nursing, Physiotherapy or Occupational Therapy interventions) for a group of GP Practices as part of the Integrated Care Team. The post holder will have responsibility for managing co-ordinating and delegating work ensuring referrals are dealt with according to clinical need, Holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes. The post holder will be required to work with a high degree of autonomy and professional accountability. A primary aim of the role is to prevent unnecessary admission of patients to an acute hospital and support timely discharge into the community. The post holder will also have a responsibility for re-enablement, rehabilitation, health protection and promotion programmes that improve and reduce inequalities.
