We are looking for an experienced practice nurse with respiratory experience to join our experienced and supportive nursing team.
We would like you to have experience in primary care and of working autonomously providing management care in long term conditions in particular respiratory experience with an Asthma and COPD disease management qualification
Our practice nurses undertake the full range of treatment room and chronic disease management nursing, working closely alongside the wider practice team which includes GPs, HCA's, Phlebotomists, First Contact Practitioners, Clinical Pharmacists and our Social Prescribing Team.
We also see patients in their homes for vaccinations and reviewing their long term conditions.
About usOur nursing team undertake the full range of treatment room including wound care, immunisations and vaccinations, travel advice, cytology, phlebotomy and disease management both in practice and in patients homes. We also undertake reviews via the telephone.
