In addition to clinical responsibilities within the practice, the nurse will also provide care at patients' homes for vaccinations and long-term condition reviews, including consultations over the phone. The nursing team is involved in various tasks, including wound care, immunisations, travel advice, cytology, and phlebotomy, thus offering a dynamic work environment.
We are looking for an experienced practice nurse with respiratory experience to join our experienced and supportive nursing team.
We would like you to have experience in primary care and of working autonomously providing management care in long term conditions in particular respiratory experience with an Asthma and COPD disease management qualification
Our practice nurses undertake the full range of treatment room and chronic disease management nursing, working closely alongside the wider practice team which includes GPs, HCA's, Phlebotomists, First Contact Practitioners, Clinical Pharmacists and our Social Prescribing Team.
We also see patients in their homes for vaccinations and reviewing their long term conditions.
