The University Hospitals Dorset NHS Foundation Trust is seeking an Advanced Clinical Practitioner (ACP) for the Same Day Emergency Care (SDEC) clinic within Older People’s Services in Poole. This role is integral to providing timely and effective interventions for elderly patients, focusing on delivering comprehensive, compassionate, and patient-centered care. The ACP will manage complex cases, conduct comprehensive geriatric assessments, order and interpret investigations, and develop patient-focused management plans.

Candidates must possess an accredited MSc in Advanced Clinical Practice or completed the e-portfolio route and be committed to completing a local competency booklet. An active UK NMC Registration is required for this position. The role is based at Poole Hospital, and further details can be found in the attached job description and person specification.
