The Home Care Practitioner Coordinator position at North Cumbria Integrated Care NHS Foundation Trust in Cleator Moor is aimed at Occupational Therapists, Nurses, or Social Workers. The role involves overseeing a team of approximately 14 home care practitioners to facilitate the safe and effective discharge of patients from hospitals and to provide ongoing support in their homes, thus preventing readmissions. The Coordinator will be responsible for assessing patients prior to discharge, creating individualized care plans, coordinating care across various professionals, and ensuring that patients' needs are continually assessed and met during a two-week service period.

As an autonomous practitioner, the Coordinator will manage a defined caseload and work collaboratively with Adult Social Care and community teams to develop and monitor outcome goals tailored to each patient's health and social care needs. The position emphasises promoting patient independence and safe transitions from hospital to home, with particular attention to the national discharge policy and preventive strategies against unnecessary hospital admissions.
