Gloucestershire Health and Care NHS Foundation Trust is seeking a Community Mental Health Practitioner to join their Crisis Resolution and Home Treatment Team in Cheltenham. This team plays a vital role in urgent care services by providing triage, assessment, home treatment, and telephone support for individuals experiencing psychological, emotional, and mental health distress. The ideal candidate will be a qualified RMN nurse committed to making a positive impact on patients' lives and capable of working in high-pressure situations. Responsibilities include conducting comprehensive assessments, offering short-term interventions, and maintaining a shared caseload of 20 to 30 service users.

The position requires effective communication skills and the ability to collaborate with key partner agencies to ensure an integrated approach to care. The team operates on a flexible shift system, offering both short and long shift options, and supports staff with regular supervision from senior clinicians. The successful candidate will help provide a 24/7 mobile community service as an alternative to hospitalisation, aiming to facilitate crisis resolution and support service users in maintaining their wellbeing in the community.
