To support the Consultant in Public Health and Population Health lead in the delivery of the Population Health strategy and yearly work plans.
Working closely with the Population health delivery group and stakeholders from across the local system in a range of communities and organisations.
To be responsible for the co-ordination and project management of the Population Health work plan.
To develop and maintain excellent communication across work areas and between different programmes of work.
To act as an ambassador for the Population Health Department agenda for example Health inequalities, Sustainability, Prevention and Anchor Organisations.
Co-ordinate and engage with local communities and a range of internal and external professionals in relation to the Population Health Department agenda.
To support the evaluation of the Population Health agenda and yearly work plans.
Comply with the Trusts vision, values and behavioural compact.
This is a fixed-term post until 30th November 2025.

This is your chance to help deliver the new Population Health Strategy across South Tyneside and Sunderland NHS Foundation Trust (STSFT).
You will work with a range of partners across the local system to deliver key Population Health projects. To start, you will focus on improving health literacy in the local area.
The role would suit someone who cares about fairness and tackling unfair differences in health. You should be highly motivated and enjoy working in a changing environment. This is your chance to make a positive difference to our local communities.
You will be responsible for the co-ordination and project management of key Population Health projects. To start, the focus will be health literacy projects in the local area.
You will work with the Health Literacy Officer, Consultant in Public Health, the Population Health Lead and a small core team of staff based at South Tyneside Hospital. You will also work closely with our local communities and system partners.
You must be well organised and adaptable to manage the different parts of this role. Project management and quality improvement skills are helpful. We want someone who communicates well and is a team player. You also need to be a self-starter and problem-solver.
Experience of working on large scale projects and delivering to deadlines is important. You must have experience working in a complex system with a range of stakeholders.
You will promote the Population Health Department agenda. This currently covers health equity, prevention, sustainability and anchor work programmes. You do not have to be an expert in any of these areas to apply for this role. But you should have experience of working to tackle health inequalities and be committed to the principles of fairness and social justice.
