The Community Learning Disability Nurse position in Oldham with Pennine Care NHS Foundation Trust involves joining a multi-disciplinary integrated team within the Learning Disability Directorate. The role focuses on providing specialized health and behavioral assessments to individuals with learning disabilities, developing personalized care plans, and delivering interventions in various settings, including clients' homes. The nurse will coordinate clinical care, manage risk, and work collaboratively with social services and safeguarding teams to ensure effective, safe, and person-centered care. Additional responsibilities include mentoring students, participating in team development, and engaging with families and stakeholders to promote good health and inclusion for individuals with learning disabilities.

Candidates should possess experience in learning disability nursing and be adept in undertaking comprehensive assessments for individuals with complex needs.
