The role of Triage Practitioner for Urgent Emergency Care at Granta Medical Practices in Cambridge involves working within the UEC Hub to manage patient referrals for urgent care, ensuring timely and appropriate care for adults in the Cambridgeshire and Peterborough area. The successful candidate will assess and triage incoming referrals, providing clinical expertise and decision-making to prioritize care based on various factors affecting patients' health. Responsibilities include coordinating a multidisciplinary team (MDT) to facilitate community admission avoidance pathways, supporting patients for up to two weeks, and maintaining accurate clinical records in alignment with regulatory guidelines.

The position requires an experienced Registered Nurse or HCPC Professional who will collaborate with multiple health and care providers to prevent unnecessary hospital admissions and ensure patients receive care in the most suitable setting. Working hours include shifts throughout the week, with the UEC Hub operating from 8 am to 8 pm, every day of the year. The practitioner will play a key role in enhancing the system's responsiveness and effectiveness by contributing to service evaluation and development.
