The UEC Hub operational team, oversees the delivery and function of the UEC Hub single point of access (SPoA) on behalf of the Cambridgeshire and Peterborough ICS. This will include the provision and management of the system UEC Hub Trusted Assessors.
The service will operate 7 days per week from 8am 8pm; however, these hours are subject to review in collaboration with the ICS and provider. The Hub will be accessible to Healthcare Professionals and will accept urgent/same day response referrals for patients who are registered with a Cambridgeshire and Peterborough GP Practice, and aged 18 or over.
The UEC Hub is a partnership between community, ambulance, primary care, acute services and social care that brings together a multidisciplinary team (MDT), that can prevent avoidable ambulance dispatches and conveyances, emergency department (ED) attendances and hospital admissions. Ensures timely transfer of appropriate patients from ambulance stack into alternative receiving services by improving interoperability, for example implementing interoperability toolkit messaging as a minimum
This requires-
visibility of patients waiting for an ambulance dispatch
access to shared records
ability to transfer a case between MDT members as required
visibility of appropriate waiting lists and real-time capacity
ability to book directly into and referral to key services
Undertake clinical assessment and triage of referrals coming into the hub, supporting the single point of access into our community admission avoidance pathways which may include health, social care and voluntary sector services.
Provide high level clinical expertise and decision-making regarding patient care including prioritising factors involved in the presenting conditions through clinical reasoning based on knowledge of current professional opinion, clients prognosis and other physical, social, emotional and mental health issues.
Working as part of an MDT to hold responsibility for the day-to-day co-ordination of management of referrals and onward signposting to the relevant services.
Support the case management of patients for up to two weeks, until the patient is fully discharged (either onto other services or where no further input is required)
Working with providers to understand capacity and capabilities, to ensure patients are seen in the right place at the right time based on their clinical presentation/needs.
Record accurate, up to date and evidence based clinical records in accordance with the legal framework, NMC/HCPC guidelines and local record keeping guidelines.
Keep up to date with evidence based clinical information and propose changes to local and organisational practice as appropriate; supporting the UEC Hub Operational Lead to develop, deliver and evaluate the service and assist in any further developments.
About usThe post holder will be an experienced Registered Nurse in Adult Nursing or HCPC Professional who will work within our Urgent Emergency Care (UEC) Hub, supporting the Cambridgeshire and Peterborough single point of access for our Urgent Community Response (UCR) and wider admission avoidance pathways. This is an integral part of the out of hospital care model designed to ensure that patients receive appropriate care and treatment as close to home as possible.
The hub receives referrals from a range of health and care professionals which require triages and assessment to determine the right care at the right time for the patient to ensure they can remain safely at home. As Clinical Trusted Assessor, you will provide community based responsive input to patients who are referred into the Hub to ensure holistic assessment and community care pathway planning is undertaken. You will be responsible for assessing and evaluating care for adults with the aim of preventing inappropriate hospital admission and facilitating onward referral to the most appropriate service, including our UCR 2-hour response teams who can provide up to five days of care in the community.
In a small number of cases, it will be appropriate for the Hub to lead the case management of a specific patient for a short period of time, for example, where a patient required support from more than one onward service.
You will work closely as part of an MDT to understand/support capacity, demand and referral priorities to ensure patients are seen in the right place at the right time in line with agreed Service Operating Procedures.
