An exciting opportunity has arisen for a self-motivated individual to join our Epilepsy Team. This role will be primarily based at our Great Horton surgery but will require travel to our other sites and various locations across Bradford/Airedale.

  2. Communicate care needs with patient, family members and relevant professionals.
  3. Liaise with primary & secondary care teams as needed.
  4. Participate in regular team meetings with the GPs with extended role in epilepsy and secondary care consultant.
  5. Actively participate in peer support network with epilepsy nurse specialists.
  6. Sign posting & supporting patients, families & clinicians where appropriate.
  7. Offer high quality care to patients with epilepsy.
  8. Enable MDT approach to care of patients with epilepsy.
  9. Facilitate an efficient & accessible service for epilepsy patients.
