We have an exciting opportunity to join a wonderful, dynamic and patient focused team providing high quality services to our local community.
This role will provide you with the opportunity to deliver a high standard of care to patients in their homes alongside developing your leadership skills supporting your team.
Could you be our new Senior Staff Nurse looking for a new challenge?
Hours: 37.5hours per week (including weekends, bank holidays, nights and on-call)
The successful candidate will work autonomously and as part of a team contributing to the provision of 24/7 Specialist Palliative/End of Life Care. They will be responsible for not only providing care, but for providing advice and support to patients, their families and other healthcare professions within the community.
The postholder will provide support to patients in their own homes or preferred place of care who have palliative or end of life care needs, raising clinical standards and promote a learning environment for other health care providers. They will be responsible for planning, assessing, implementing, and evaluating care in partnership with the Multidisciplinary Teams and out of hours providers.

KEY TASKS
To provide an environment that encourages patients and their families to play an active part in their care enabling them to voice their wishes about current and future care requirements
To provide advanced levels of clinical decision-making through assessment and interpretation of clinical information to support patients, families and carers who have complex or rapidly changing physical symptoms, emotional or practical needs
To assess, plan, implement and evaluate patients care needs as required by your designated patients, including admittance to the Hospice/Hospital and advanced care planning including identifying their preferred place of death
To provide appropriate pain and symptom management and provide directional support for their psychological and spiritual needs of the patient, their families, and carers
To provide support, information and advice to patients, families, colleagues, and other healthcare professionals, including supporting the Mountbatten Care Co-Ordination Centre
