We are looking for a School Nurse with Specialist Community Public Health Nursing (SCPHN) qualifications, to join either our public health or safeguarding team delivering the 0-19 Healthy Child Programme.

You will need to have a broad understanding of children and young people's health issues and be able to demonstrate flexibility and creative thinking in respect of service delivery. Knowledge of recent documents and strategies driving the development of universal and equitable services for children is essential as well as understanding the principles of safeguarding children and young people.

You will be the named School Health Nurse for an identified High School, college, and a cluster of Primary schools. Manage the defined caseload, using a public health approach to provide and maintain a high-quality service that promotes the physical and mental health of the school aged population and their families.

Lead the delivery of the Healthy Child Programme 519 and co-ordinate the delivery of public health interventions for the post holders assigned schools under the direction of the 0-19 Team Leader. This will include health screening/surveillance health assessment, immunisations, and a requirement to support the health needs of any vulnerable children/young people within the caseload.

About us

  1. The named School Nurse will be responsible for co-ordination of the School Health Plan with their named schools on an annual basis.
  2. Contribute to an integrated Public Health Nursing Service by linking to education settings and other partner agencies
  3. Lead on the recommended health assessment and reviews including:

Reception/Year 1 (age 45) school entry assessment (transition from 05 HCP school entry questionnaire; Providing Year 6/7 (age 1assessment at transition from primary to secondary school.

  1. Supporting mid-teens reviews, when young people are embarking on the next transition stage as and when required in national and local specifications.
  2. Lead and coordinate a core programme of interventions according to agreed procedures, ensuring that Trust targets are met.
  3. Undertake comprehensive health assessments for identified children and young people, including annual health assessments for those in local authority care in line with the Healthy Child Programme 4-19 years of age.
  4. To contribute to the delivery of the national immunisation programme for school aged children.
  5. Plan, implement and evaluate interventions according to changing health needs to improve health outcomes. Consult, advise and educate children, young people and families to enable them to effectively manage their health needs.
  6. Perform home visits to children and families advising on all aspects of health as appropriate.
  7. Working in partnership with other key stakeholders to safeguard and protect children and young people.
  8. To contribute and take part in regular team meetings and contribute to multi-agency groups as appropriate.
  9. Working collaboratively to support children and young people where there are identified health needs, or where they are in the child protection system, providing therapeutic public health interventions for the child and family
  10. Provide training where necessary to young people, parents, teaching personnel and other professionals.
  11. Provide signposting to other services and agencies, referring and liaising appropriately.
  12. Take and act upon referrals from other agencies and services, as appropriate.
  13. Provide a confidential support and advice service; that is easily accessible, for young people within a high school or other venue on a regular basis.
  14. Organise, facilitate and participate in health promotional and health educational activities in schools and community settings, working in partnership with school staff and with other statutory and voluntary agencies.
  15. Utilise opportunities to promote healthy lifestyles, positive mental health and the public health agenda.
  16. Ensuring children not in employment, education or training, or children educated at home receive the universal offer.
  17. Complete PDRs and return to work pro-forma as required to the designated named staff within the team.
  18. Ensure performance data is collated in a timely manner


Clinical



  1. Maintain active status on NMC register acting in accordance with the professional code and guiding documents in adherence with Revalidation Framework.
  2. Develop a working knowledge and experience of IT systems
  3. Maintain contemporaneous, factual records in accordance with Trust guidelines and policies.
  4. Be aware of current working practices in accordance with Trust policies and procedures.
  5. Manage their time effectively to ensure prioritisation of workload in order to achieve agreed deadlines.
  6. Be responsible for own professional updating and keep abreast of current research based practice.
  7. Participate in study days as appropriate and be aware of current developments and research within the area of practice.
  8. Observing the complaints and disciplinary procedures in accordance with the Trust policy.
  9. Report sickness absence and return relevant forms to line manager.
  10. Maintain portfolio of professional development.




Leadership



  1. Provide supervision, where appropriate to support the skill mix team
  2. Undertake appropriate delegation within team.
  3. Support other School Nurses and staff within the team, including the induction of new staff members. Ensure effective learning experience and opportunities to achieve learning outcomes for skill mix team.
  4. Develop specialist knowledge and expertise in an area of practice appropriate to service needs and, act as a resource to colleagues and other professionals in the chosen area e.g. Asthma, Behaviour Management. Allergy etc.
  5. Develop and implement policies, procedures and guidelines relevant to own area of work. Provide advice where services are developing.


Research and Audit



  1. Identify and undertake clinical audit, evaluation and dissemination of results through presentation to professional groups and publications, in order to improve effectiveness of care and ensure effective and efficient use of resources.




Supervision and Development



  1. Identify and make known any training needs through the Personal Development Review (PDR) process.
  2. Provide mentorship and preceptor ship to students and newly qualified staff as required.
  3. Undertake mandatory and statutory training and updating as required by the Trust.




Communication



  1. Communicate effectively with individuals, families and professionals in planning and delivery of care, demonstrating empathy and reassurance in difficult and highly sensitive situations.
  2. Maintain contemporaneous and accurate Bridgewater Community NHS Trust records observing the Data Protection Act 1994, maintaining confidentiality, in accordance with NMC and Trust guidelines
