An exciting opportunity has arisen for an experienced cardiology nurse to be part of the heart failure team at UCLH. We are looking for someone who is driven, enthusiastic and innovative with excellent experience in the care and management of patients with heart failure. Flexibility across the cardiology specialism will be required to support services and provide cross cover.
Fundamental to the role is working closely with the Heart Failure Consultants and the wider cardiology and medical teams and ward staff. Weekly multidisciplinary team meetings, plus working alongside consultant colleagues, will provide the post holder with ongoing access to expertise and professional support. Clinical duties include referral triaging, participating in inpatient ward rounds, conducting outpatient consultations and remote management of HF patients. These duties will be undertaken in close collaboration with cardiology consultants and advanced nurse practitioners.
The post holder will be expected to run one-stop heart failure clinics on a weekly basis and support the UCLH hospital at home team in managing heart failure patients.
This is a fixed term contract with the aim to become substantive after 12 months (conditional). This post requires a five-day week commitment (37.5hr) without night, weekend, or bank holiday working. There are no home visits involved.We highly suggest getting in contact for an informal discussion or visit on site.
