Following a retendering process, the children's service is due to be transferred to the new provider on the 1st of April 2025. Given this post sits within the transferring service, in line with the legislative requirements, the successful candidate will be part of the TUPE transfer along with other colleagues.

This is an exciting challenge for a motivated and innovative Health Visitor to join our experienced skill mixed 0-19 team based in Dorking, Leatherhead, Banstead and Tattenham.

The ideal candidate for our team would be a clinical practitioner with leadership and excellent interpersonal and communication skills. We look for individuals with a strong approach to teamwork, those who demonstrate initiative, a good understanding of children, young people and their families and ability to support families from a diverse population within the borough.

About us

The post holder will be a member of the 0-19 public health team delivering an evidenced based and needs led service for children, young people and their families in line with the Healthy Child Programme (HCP). Supported by the clinical team lead, the post holder will carry continuing responsibility for the assessment of health needs, including the development, implementation and evaluation/audit of evidence based interventions.

She/he will take responsibility for ensuring the quality and standard of care and delivery of a service to improve outcomes for children and families. The post holder will participate in delivering a universal core service, and provide targeted interventions, to address health inequalities, where necessary referring to specialist services. This role will also include safeguarding responsibilities, mentorship of students (pre and post registration), line management of a skill mixed 0-19 team and staff appraisals.

Principal duties to include:

  • Work collaboratively with the skill mixed health visiting team to ensure equity ofservice provision, promoting good practice and the sharing of resources.
  • Prioritise own work in line with areas of highest risk or client groups with greatesthealth need, delegating other work to the wider skill mixed team whilst providing theteam with supervision and support
  • Facilitate good team working, through effective leadership skills delegating andsupervising the work of a skill mixed team to ensure efficient delivery of the 0-5HCP
  • Assist in meeting public health targets e.g. teenage sexual health, accidentprevention, breast feeding, immunisation, smoking cessation and obesity
  • Undertake continued responsibility for the management of a caseload
  • Build and develop good working relationships with childrens centres, midwifery, GPpractices and the wider multi-disciplinary team.
  • Work collaboratively with all relevant agencies to influence/develop policies whichwill enhance health
  • Be responsible for organisation, prioritisation and planning of health promotionactivities, by assessing local health needs and promote key health topics to meetAll our health 2DH targets
  • Where appropriate work with Children and Family Health Surrey (CFHS) Nurseprescribing lead to prescribe in line with local and national policy guidelines.
  • Make Every Contact Count with children, and their families in a variety ofcommunity settings e.g home, children centres, nurseries
  • Undertake well child clinic sessions within agreed venues and children centres
  • Assist in the planning and where appropriate in the delivery of continenceassessments
  • Identify and support children and families who have unmet health needs, providingsupport and targeted interventions and where necessary referring to specialistservices
  • Participate in prevention and early detection of all categories of abuse in all agegroups
  • Take appropriate action in cases of suspected or actual child abuse, working withinSurrey safeguarding childrens code of practice and procedures
  • Attend child protection conferences/core groups as required and contribute to thedevelopment of the protection plans and decision making processes, within themulti-agency setting
  • Participate in safeguarding supervision
  • Ensure that clear concise reports are written and that organisational record keepingguidelines are adhered to
  • Assist in research, participate in clinical audit and evaluation of service includingthe monitoring of records of others within the 0-19 team
  • Undertake health needs assessments for children who are looked after
  • Support and attend group clinical supervision as per organisation policy
  • Actively participating in steering groups to develop HV practice guidelines
  • Supervise junior members of the skill mixed 0-19 team as directed by the ClinicalTeam Lead and undertake junior staff Personal development Review (PDR) appraisals and direct line management.
  • Contribute to the pre and post registration opportunities made available to studentsacross CFHS, which may include supervision of student practice placements
  • Assess practice competencies of junior staff members, contributing to their trainingand development needs
  • Participate in meetings as appropriate
  • Be prepared to be flexible within contracted hours
  • Work in accordance with the needs of the service, undertake other duties asrequired within current skill set
