The Hospital at Home Lead Pharmacist for Manchester University NHS Foundation Trust is a part-time, Band 8a role focused on enhancing home-based patient care and medicines management services. The ideal candidate will be an experienced pharmacist with advanced clinical knowledge, capable of working collaboratively within a multidisciplinary team to optimize medication management, reduce hospital re-admissions, and improve patient safety and compliance. Responsibilities include leading the medicines optimisation support for the Hospital at Home (H@H) program, supervising pharmacy staff, providing specialized advice, and implementing new services and policies aligned with local and national guidelines.

Applicants must be registered with the General Pharmaceutical Council (GPhC), hold a postgraduate clinical diploma or equivalent, and possess significant experience in hospital or primary care, ideally as an Independent Prescriber or in progress toward that designation. The role also involves supporting the Community Medicines Optimisation Service and contributing to the training of Local Care Organisation staff.
