The West Kent Health & Care Partnership (West Kent HCP) focuses on the implementation of the West Kent HCP transformation portfolio objectives.

The thriving West Kent Health & Care Partnership (WKHCP) are pleased to advertise an exciting opportunity for a Transformation Programme Officer. The post holder will be part of the highly regarded Joint Programme Management Office (JPMO) that supports the delivery of our WKHCP cross organisational transformations.

WKHCP is a collaborative, area wide partnership between the full range of West Kent health and care providers and commissioners, including: Maidstone & Tunbridge Wells NHS Trust, Kent Community Health Foundation Trust, Kent & Medway NHS & Social Care Partnership Trust, West Kent Primary Care, Kent County Council and 4 district councils (Maidstone, Sevenoaks, Tonbridge and Malling, Tunbridge Wells), community and voluntary organisations, local resident forums and the Kent & Medway Integrated Care Board.

West Kent HCP is implementing an ambitious programme of transformation to support integrated working across West Kent. The partnership has prioritised the following areas for change: Integrated Neighbourhood Teams, Health Inequalities, Discharge and Flow, Frailty and Adult Complex Care and Children and Adults Mental Health.

The West Kent HCP is a collaborative, system wide initiative between the full range of partners in West Kent including: Maidstone & Tunbridge Wells NHS Trust (MTW), Kent Community Health Foundation Trust (KCHFT), Kent & Medway NHS & Social Care Partnership Trust (KMPT), West Kent Primary Care (WKPC), local authorities (Kent County Council/district councils), community and voluntary organisations, local residents and the Kent & Medway Integrated Care Board (ICB). Our whole system priorities for the West Kent area mean that the West Kent HCP Joint Programme Management Office (JPMO) team works across all partner providers and organisations, and this role, which is primarily supporting the West Kent HCP Director, will also interface with the full range of partners.

This post will report to the Head of West Kent HCP JPMO, who is the leader of the programme team supporting the delivery of the West Kent HCP transformation priorities
