MENTAL HEALTH MULTI-DISCIPLINARY NHS
The Band 6 Primary Care Mental Health Practitioner position in Solihull with Birmingham and Solihull Mental Health NHS Foundation Trust invites enthusiastic individuals to join the Solihull Neighbourhood Mental Health Team. This full-time role involves providing mental health triage, assessment, and outcome-based interventions in alignment with the NHS Long Term Plan. The practitioner will work collaboratively within a multi-disciplinary team, including GPs and other primary care professionals, to enhance the psychosocial well-being of service users and their families by identifying needs and facilitating appropriate pathways for support.

Key responsibilities include assessing mental health needs, delivering interventions, and promoting communication between mental health providers, local community organisations, and primary care. The team is focused on evolving primary care mental health services as part of a broader community transformation initiative, making this role pivotal in contributing to national changes in mental health care delivery.
